• Sports Physical - KSHSAA form, fill, save and print PDF, take to doctor to complete if your student is going to participate in KSHSAA athletic events (football, volleyball, basketball, track, tennis, wrestling, cheerleading, cross country)

COLLEGE PERMISSION

• Dual/Concurrent Credit Permission Form - complete if a Junior or Senior student is planning to take any college credit courses

FOR SENIORS ONLY:

Name on Diploma - fill out with student's legal name as it should appear on the diploma

• Supply Lists Pre-K through 6th grade
• Supply Lists grades 7-12
• Request for Records - fill out PDF, print and sign then return to grade school or high school office
• Out of District- if you live out of district, please fill out this form
• Parent Online - form to complete for permission to access to the student information system with your student's grades, assignments and attendance

Other Documents You May Need

• Medication Form - if your student takes a prescription during the school day, the nurse must have this form completed and on file in order to administer your child's medication - fill out PDF, print and sign and then return to nurse
• Immunization Letter - please read - all students are required to be fully vaccinated unless you have a religious exemption (see the document for religious exemption)
• Religious Exemption Immunization - If you have a religious reason for non-vaccination of your child, complete this form, print, sign and return to grade school office

• Free-Reduced Meals - fill out form, save, print and then sign and bring to District Office
• Meal Modification - Meal modifications prescribed by a medical authority 
• Migrant- Parent Survey

• Free Textbook Rental Application - if you are on public assistance and would like to apply for free textbooks
